Supplemental Educational Services (SES) include summer and before- and after-school tutoring in reading, language arts, or math. They may be offered through public- or private-sector providers who are approved by the state, such as public schools, public charter schools, local education agencies (local school systems, herein referred to as LEAs), educational service agencies, faith-based organizations, and community groups.
Students from low-income families who remain in Title I schools that fail to meet AYP for at least three years, and are thus in the second year of Needs Improvement status, are eligible to receive SES.

Program Summary
The Director of Get Smart Consulting has over 24 years as an educator providing instructional strategies to improve the academic success of students. The program will utilize these proven strategies to encourage these same results in our students. We promote the incorporation of hands-on experiences and instruct tutors on using a Learning-Focused approach which utilizes graphic organizers, daily assessments, and writing to promote comprehension. This approach has proven highly effective with varying at-potential populations. Materials used will include workbooks correlated to Georgia Standards and additional resource materials which allow for adequate practice of skills. Get Smart Consulting's program is flexible and accessible to meet the needs of parents. Services are provided at school, in community locations, and in-home as approved by the director. Tutoring may be held between the hours of 3:00 p.m. - 9:00 p.m. weekdays and up to 3 hours on weekends. Tutoring may be conducted 2-3 days per week. Transportation is provided for select locations. Get Smart Consulting will work with the parent/school to ensure that the instructional goals set are in-line with areas of concern. Interviews will be conducted with parents prior to service; available school documentation will be reviewed, and pre-testing results will be analyzed. On-going benchmark testing will be utilized to determine retention of instruction and the need for re-teaching. Although Get Smart Consulting had provided SES services for one year, It is important to note that our Director has worked with other companies and LEA's providing services for 8 years.

Program Summary
Learning Solutions, has been serving students in the Atlanta area for 19 years. The longevity, the return of students, and the improvement of academics demonstrate the effectiveness of the tutorial program. The UGA/DOE 2007-2008 survey analyzed the test scores of students served by Learning Solutions and showed that in math, more than fifty percent of the Learning Solutions SES students scored higher on the State CRCT than the comparison group. The Survey showed that in reading, a greater percentage of Learning Solutions SES students moved to a higher performance level on the State CRCT than the comparison group. Tutorial sessions are 1.5 hours (bi-weekly) with choices of days and times (Monday-Thursday, 4:00 p.m. to 8:00 p.m. / Saturday 10:00 a.m. to 12:00 p.m.). Flexibility is necessary. Parents who are the sole source of transportation often have schedules that create hardships. These schedules must be addressed. The instruction at Learning Solutions is based on the results of a reading/L.A. and math Basic Skills Inventory given to each student and the review of each student's CRCT. Using this information, the student is prescribed an individualized curriculum with goals set to improve weak areas as shown on the BSI and CRCT. The curriculum is designed to improve grades, CRCT scores, and aligns with GPS. An integrated approach is used in reading, to address word analysis, vocabulary, comprehension and study skills. The math curriculum utilizes a balanced mathematics approach where basic knowledge, operational skills, and the ability to make application are equally fostered. The curriculum is computer based and monitored by teachers with advanced degrees who are continually helping the students (5:1 ratio). Learning Solutions is located in a quiet, accessible plaza. There are 18 computer stations and 5 work stations. All instructional materials are provided. Learning Solutions has been a Georgia SES provider for seven years.

We believe if a child attends our program regularly, we can help them. We have over 35 years of tutoring experience and our program is designed with the highest regard for helping struggling learners. We tutor students in Kindergarten through 8th grade in reading and math. Our professionally trained tutors, most of whom are certified teachers from the local school and community, are trained to ensure that our students are always supervised. Our program is housed mainly in the local schools with local teachers, paraprofessionals and qualified staff hired to do the tutoring. Our classes are held two days a week for 60 to 90 minutes each day, and we tutor in small and large groups. A key component of our program is the goal setting conference which allows all parties; our staff, parents and the school personnel to participate following an examination of standardized test scores, other school information, or diagnostic pre-test, if needed. With this information we develop a Student Learning Plan with specific goals and individualized curriculum for the student. To ensure scope and sequence of skills taught through tutoring, our curriculum is aligned with the Georgia Performance Standards (GPS), the child's classroom curriculum, and the master plan of each School District we serve, while employing a variety of research-based strategies. Our program emphasizes 5 Reading strands and 5 Math strands. Frequent assessment and reporting progress are key components of effective teaching and we present a progress report monthly to parents, teacher and school districts. Our company administers a post test (Achieve) to determine student growth and curriculum strengths and weaknesses. A to Z In-Home Tutoring provides in person, one-on-one instruction in the comfort and convenience of a student’s home or in small groups of 3-6 students at school or alternate location. Our tutors work with children in Reading, Language Arts and Math. As one of the largest national providers of No Child Left Behind tutoring for the past seven years, we are very experienced, yet have a personalized local approach, as we have operated in Georgia since 2004. In 2006-2007, students enrolled with A to Z experienced an average gain of 4 newly acquired math and language arts skills. All of our curriculum content is research-based and focuses on helping struggling students succeed. Students are pre-tested to identify which skill sets they are deficient in so tutoring services are 100% custom tailored and individualized for the needs of the client. Periodic mini-assessments are used to monitor the student’s growth. During the final session a post-test is given to measure the overall gains of the child and the new academic skills they have achieved. Through traditional one-on-one and small group tutoring, students are able to reach and exceed their educational goals much more quickly and effectively. Strong teaching backgrounds and a comprehensive understanding of children’s needs enables these qualified tutors to develop learning skills, polish self-esteem, boost confidence and enhance motivation. The student’s family and tutor determine the tutoring schedule. Program Summary
A.Z. Kelsey Academy will provide supplemental services in the areas of reading and math to middle school students. Housed in a former middle school facility, A.Z. Kelsey Academy will serve as the primary site with other school sites identified throughout the system. Both external and internal providers have offered supplemental services in the Grifin-Spalding County School System for over five years. The effectiveness of the supplemental services delivery model is evident through the academic achievement of middle school students enrolled at Kennedy Road Middle School (KRMS). After seven years of being in needs improvement, at the end of the 2005-2006 school year, KRMS, formerly A.Z. Kelsey Middle School, was the only middle school in the system that made AYP. Students at the school, especially subgroups, demonstrated remarkable improvement in key content areas. The school has maintained AYP status for the past two years and consequently, has come off the state's Needs Improvement listing. This proposal calls for supplemental services to be delivered in two ways: tutorial sessions on Tuesday and Thursday afternoons for 90 minutes a day (from 3:30 - 5:00) and Saturday morning tutorials for 3.5 hours lasting from 8:30 - 12:00 with a 30 minute break. An additional hour will be added to each day for teachers to allow time for data analysis and planning. The models of instruction included in this proposal are READ 180 for reading support and ALEKS for math support. Both programs are supported by empirical research, closely align with state and national standards, and include comprehensive on-going assessment components. A.Z. Kelsey Academy and all other sites utilized in delivery of supplemental services have fully equipped computer labs for use during individualized instructional support. A.Z. Kelsey Academy has no prior years of service as a Georgia SES provider.

Program Summary
A+ Academics (A+) is an SES provider in Oklahoma and Texas. We have successfully tutored 4,000+ students over the last 7 years. In 2007-2008, A+ serviced 900 SES students, most being underachieving and low income students. We are expanding our services and are now licensed to begin operating in Georgia.
We provide individual and small group tutoring in reading, language and math, including algebra and calculus. We offer tutoring in public facilities including libraries, churches and schools. In-home tutoring is also available. Our program uses research-based strategies in neurolinguistics programming, memorization techniques and brain-based learning. We agree with L.A. Hurt's learning theory that the educational environment should consist of four features: 1) high expectations; 2) non-threatening ambiance; 3) a 100% mastery goal; and 4) an air of reality (exposure to the same concept in more than one setting). Our program is closely aligned with the Georgia Standards and Criterion-Referenced Competency Tests and is designed to increase K-12 student academic scores.
A+ tutors are certified teachers, professionals, and college students with 2+ years experience. We work with students, parents and teachers to develop an instructional program with the following elements designed to help each student meet or exceed state standards:
1) Pre-Tutoring Assessment; 2) An Individualized Student Learning Plan (a road map); 3) Measurable Goals; 4) Progress Reports;and 5) Post-Tutoring Assessment / Final Report.
Tutoring Sessions are 60-90 minutes, twice weekly. Students are tested every 10-12 hours to measure skill development. Program Summary
A+ Tutoring is a new division of Challenging Failure expanding to provide the same quality instructional program to high school students. Our program uses technology to identify where the student needs help and provides instruction in small groups with teachers who are trained to find the best way to teach your child. And we keep teaching until the student has learned what they need to know! Together we set goals based on the standards and a timeline is established. According to the State SES Satisfaction Survey 2008, 100% of our students believe our tutoring improved their grades and 75% of parents would use our services again. We meet in public libraries, Boys & Girls Clubs, and church halls for 1 to 2 hours per session, one to two times per week according to your schedule. Program Summary
ABOUTFACE! Reading has been teaching reading to students throughout the Southeast for 11 years and has been an SES Provider in Georgia for four years. Our reading lessons are designed using the five scientifically-based strategies recommended by the National Reading Panel to increase reading skills. Every student is pre-tested to determine their unique needs and an Individualized Learning Plan is developed with teacher and parent input. Then, leveled books are chosen that reflect each student's current reading abilities. As students' mastery increases, additional tests pinpoint remaining skill gaps. Learning Plans are then updated to fill those gaps. Finally, post-tests are administered to gauge student improvement. Each child receives approximately 20 hours of tutoring, usually 1 hour sessions, twice weekly. All of our pupils will move to the next reading level or higher. Every child keeps every book they read. Lessons are held in LEA's, churches, libraries or other public area. We provide all materials including scripted lesson plans, books, word cards, and any other materials needed for the course of instruction.

Program Summary
Custom One-on-One, or small group tutoring, (less than 8:1), in the student's home or pre-arranged facility. Progress occurs because of the Individualized nature of the program, while supplementing classroom work. Qualified tutors are "matched" to the student in Reading, Language Arts and/or Math.
Parent insight, teacher feedback, school assessment scores, and Initial Pre-testing form a program blueprint, and tutoring progresses as subject mastery is achieved. Each student receives a custom Individual Student Plan & Academic Plan of Achievement and Goals, with teacher's feedback requested to assist in preparing a program for each student. Daily Lessons are prepared for each student using Georgia Performance Standards (GPS), and regular Progress Reports are provided every 2 weeks. Using a final post-test session, test data is compared to the initial Pre-test to determine improvement for each student.
Tutoring sessions are traditionally twice per week, between 1.5-2 hours, followed by a Parent Consultation. On average, improvement is one letter grade after 3 months. Student achievement improvement has averaged over 25% in Reading; Language Arts; & Math for 4 consecutive years. 6 years proven results working with at-risk and low-income families under the "No Child Left Behind Act".

The AchieveResults™ Tutorial program was developed by a former mathematics university professor and middle school math educator, and has a proven record for improving students’ academic achievement on the CRCT! AchieveResults™ Tutorial president has been in education for 19+ years, and AchieveResults™ Tutorial has successfully tutored students in mathematics and reading in grades K – 8 for the past six years, across Georgia. Because of our small group instruction, our tutors are able to identify and work on students’ individual weaknesses, based on pre-test results.
All instructional materials are provided, and are aligned with the Georgia Performance Standards that students must master for success on the CRCT. Our research-based mathematics and reading curriculum has been effective in improving students’ mathematics and reading performance both on the CRCT and in school. The AchieveResults™ Tutorial program meets 1 ½ to 2 hours after school, twice weekly at students’ school (with principal’s permission), or on Saturdays at selected locations. Staff includes certified, special education, English Language Learner teachers, and retired school administrators. All of the instructors have math degrees and are energetic about teaching math. The program is designed for 1st -12th grades. All About Partners In Education has over 11 years of experience tutoring children in Georgia. Our company consists of four staff with degrees in education and three staff with extensive managerial experience. Our approach for instruction includes direct hands-on to students in-home, one-one-one and small groups. Our small group is one teacher for every eight students. Sessions are held in schools, community center, libraries and places of worship. As part of our program, each student will complete a pre and post assessment test. The process for assessing and diagnosing student need will be addressed through a pre assessment test along with school test scores and teacher/school input. This will determine the appropriate individualized student plan (ISP). Once an ISP has been developed, each student will receive one to two hour sessions once or twice a week to for a minimum of 20 hours. Our program uses researched based Ladders to Success- Reading and Mathematics and Coach Materials which are 100% aligned with Georgia’s state curriculum. Using a curriculum that is 100% aligned with state standards has been shown to improve student achievement. These materials are designed to provide intervention to individual student needs through scaffolding lesson. Scaffolding lessons build basic skills that prepare students for state assessments and are tailored to each student’s ISP. The student will be tested halfway through the program to determine if he/she is mastering the skill. The tutor will collaborate with the parent/guardian and student’s teacher to develop a new ISP if the scores suggest it. At the completion of the tutoring session a post test is given to determine if the student has mastered the skill and improved his/her overall score. A final progress report will be sent to parent/guardian and school.

Program Summary
Alpha Test Prep's use of Differential Instruction allows us to meet the diverse educational needs of the students we serve. We use Direct Instruction, Previewing, Acceleration, Graphic Organizers, and Hands on Instruction as a few of the instructional methods to reach our students.
We incorporate the use of CRCT sub-score results, provider administered pre-test, classroom teacher conferencing, and parent/guardian input to develop student assessment profiles. From these profiles we develop Individual Student Instructional Plans for each student. ATP's effectiveness was demonstrated in the 2007-2008 school year when 100% of our students passed the CRCT.
ATP's sessions last from 45 minutes to one hour and fifteen minutes. The sessions are structured to maximize the funding each student is alloted for tutoring.
We are not tecnology based. We do however, use computers for remediation, assessing GPS aligned internet material, and test prep. All instructional material employed by ATP is GPS aligned and NCLB compliant.

Alternatives Unlimited (AU) has been an approved vendor in several states over the past five years. According to collected qualitative data, students demonstrated a more serious approach to engaging in classroom activities and showed confidence in taking standardized tests. Parents expressed satisfaction with their children's progress and the overall delivery of instruction. Our focus has been to assist students to improve academic achievement on state standards. Our model of instruction includes small group instruction that is focused on pre-testing of all students. The optimal AU time on task for our tutoring session will last two hours per day and takes place two days per week. AU does understand, however, that a two hour session may prove to grueling for the younger learners; therefore, AU is prepared to modify delivery with shorter sessions that meet more often during the week. Our class size is typically 1:5 or specifically 1:1 for special populations. Usually all tutoring takes place at the school site if space permits. Provided instructional materials include paper, pencil, rulers, technology and others when applicable.
Goals are written based on pre-test, parent, school and LEA documentation of student data. According to collected qualitative and quantitative data, targeted students demonstrated academic growth on standardized tests. Parents receive weekly updates in either English or Spanish based on pretest assessments so that they understand the gains their child is making focused on the shared goals written at the start of the tutoring. All lessons taught to students include graphic organizers that are provided by the tutor. All supplies for students are also provided by the vendor so that students have a successful tutoring experience. AU is a new SES provider in Georgia.

August House uses highly engaging stories from around the world with colorful animations, entertaining read along files, carefully illustrated picture books and targeted practice assignments to build the specific reading skills each student needs to succeed on the CRCT. The self-paced program is aligned to the Georgia Performance Standards (GPS) for grades K-5. It can be used individually or in a small group. The program and assignments are highly interactive and guide the students to practice exactly the skills that they need to master. Students will need to have internet access and are expected to use the program twice a week for one hour at home, at a library, a community center or other facility with an internet connection. Each student is assessed throughout the program and the results are used to build and Individual Student Profile (ISP) that identifies specific strengths and weaknesses matching the GPS. Progress is measured, tracked and reported to ensure that the student is practicing the specific reading skills that he or she needs to build. When students complete a story unit, they take the book home to build their own personal reading library. After completing the program, on average the students will improve their reading skills by 10% or more on standardized tests. No transportation required. This highly engaging instructional model was developed over four years of research and testing. As a result, the program targets emerging and struggling readers who need to develop stronger skills and confidence in phonics, vocabulary, comprehension and fluency as defined by the Georgia Performance Standards.

Ava White Tutorials, Inc. has been helping students for 28 years. We have been involved with 3 school systems and 6 schools in the No Child Left Behind program since 2003. To date, we have assisted 5 of these 6 schools to make the Adequate Yearly Progress List. We have seen students in our after-school programs make significant gains in Reading, Math, and Language Arts. Many of our students advance 1-2 grade equivalents after the 30 hours of tutorials. To identify academic goals, each student is given a pre-test. Then we write an Individual Learning Plan, using the results of the testing, parent and teacher input. Students typically meet after school, at their school, with highly qualified, certified teachers who have valuable years of classroom experience. We see students in small groups, usually 3 to 6 pupils in these after school classes, and they meet 2 times a week for 1.5 hour sessions. Classes are typically held right after school, for example, 3:45 to 5:15. For students who meet at the school, transportation home is usually provided by the school or we pay for a taxi service to take the students home, after the tutorials. If parents prefer, we can see their child at our office for 1:1 tutorials and parents are then responsible for transportation. We use explicit, direct, incremental, systematic research-based instruction in teaching basic reading, language arts and math skills. Wilson Reading Program, LANGUAGE! and MATH*Ability are the main curriculums we use to help students reach their academic goals. All instructional materials are provided for the teachers and students. We do benchmark assessments throughout the program to ensure that students are making appropriate scholastic growth. Progress reports are sent home on a monthly basis, so that parents know how their child is making headway. We normally have 2 pizza parties during the 30-hour instructional period to reward attendance and celebrate student success.

Back to Basics, Inc. is going into its eighth year of providing tutorial services in the metro Atlana area, six of those years we have been offering instruction through the No Child Left Behind Program. Back To Basics, Inc. focuses on Reading, Language Arts, and Mathematics instruction for grades K-8. Our coaches work with your children one-on-one in your home or with small groups of no more than eight students in a non-competitive, nurturing environment. We offer extensive academic assessment which specifically targets your child's current academic strengths and weaknesses. Each student will then receive customized instruction based upon their individual needs. Program Summary
As the Director and owner of BridgeHaven, in operation for fourteen years, I served on the Georgia Board for the International Dyslexic Association from 1998 to 2004, and I designed our program for students struggling, sometimes physiologically, with academics, especially in reading and writing. Our practice is to structure curriculum so that needs will be met and be the primary focus. Hence, we use programs that follow the instructional model of renowned researchers in the areas of learning disabilities and dyslexia (Samuel Orton-Anna Gillingham). The Lindamood-Bell (LMB) process and programs are our umbrella programs for both reading and math. Other programs reflect these methodologies including Megawords, Touchphonics (reading) and Mathline, On Cloud Nine (LMB), Math Their Way and Summer Skills Sharpener (math). The LMB program is research-based and developed for those with learning differences that might inhibit acquisition of skills in their classroom environment. The effectiveness of the program is proven in both pre and post- test scores and in teacher/parent feedback (both to us and on SES documents). When a child can read, all skills associated with reading can be done effectively and test scores will improve. Each session is one hour and usually offered two days a week. Students can be served individually or grouped in small groups. The instruction is multisensory to meet the learning styles of each student. Skills are assessed and program placement determined based on the Woodcock Reading Mastery, Key Math and CRCT scores. The objective-based curriculum is matched to each student’s specific needs. BridgeHaven can offer services at a student’s school or students may receive assistance at BridgeHaven. Al materials are supplied by BridgeHaven. Computers are available but are used as a support tool only. We have provided SES services since 2003-2004 with the common purpose of meeting each and every child where they are and to provide success.

Bright Sky Learning has provided highly-effective, research and evidence-based K-12 reading and math remedial instruction for 7 years (3 in GA SES) in a variety of urban and rural settings, with Title I, ELL, At-Risk, SWD, and Alternative Education students. The dynamic, multi-sensory program is based on locating gaps in student knowledge through pre-assessments and designed to bring students up to or above age/grade level while increasing metacognitive abilities, confidence and self-esteem. Parents/guardians receive an orientation and are continuously informed about their child's progress and ways they can encourage and help. Student Learning Plans with specific achievement goals are aligned with district instructional programs and for each student (based on IEPs and 504 plans), collaboratively developed from assessment, school, parent and student information, and designed to meet individual student needs, in accordance with Georgia DOE standards. The program utilizes embedded assessments to continually monitor progress and post-assessments to determine achievement attained. Delivery locations are in schools, faith-based organizations or community centers accessible to eligible schools. BSL complies with the ADA requirements by ensuring tutoring locations are accessible to SWD and have adequate safety procedures. The student to tutor ratio is 5:1 and the program runs approximately 25-30 hours. Afterschool tutoring sessions are 1 or 1.5 hours, 2 or 3 times per week, or 1 3-hour session on Saturday. Independent research of the reading program has shown students gained approximately 1.5 grade levels with 20+ hours of instruction and states: "tutoring made the largest impact on the lowest performing students - students at or below the bottom 25th percentile." For math our own research shows an average grade level increase of 1.25 with 15 hours of instruction. All curriculum materials are provided.

The initial instructional program was created to improve the Georgia High School Test scores. The program is designed to target students that are at risk to fail the math portion of the GHSGT. The students have been selected by GHSGT predictor test scores as well as Teacher recommendations. The program also has been able to improve scores for students that have already taken the test but need to re-test for graduation. The program is designed to prepare the students to meet or exceed standards on the GHSGT and to become more confident math students. The focus of the program is positive re-enforcement for students that are usually not model students and often have no student support system at home. We designed the program with the concept of mastery learning. Our summer tutorial services primarily targeted 11th grade students who failed the math or English/Language Arts portion of the Georgia High School Graduation Test (GHSGT); however, there were number of 12th grade students who also attended the program and as a result were able to earn their high school diploma and increased the overall graduation rate of the school. The Burke County High School's graduation went from 61% in 2008 to 71% in 2009. It was felt that the summer tutorial program, especially the mathematics section, greatly contributed to this increase. Prior to the summer tutorial only 68% of students passed the test the first time. After the summer program this number increased by 3 percentage points which can be considered significant. Of the students involved in the summer program, 89% passed the Mathematics re-take and 45% passed the English Language Arts portion at re-test.

Over the years, C2 has compiled statistics as to how many hours of instruction it takes to raise students from a given level of performance (as measured by our initial assessment using the California Achievement Test) to a desired level set in the program goals (worked out by parent and C2 center director). While most students come on average 2 times a week, that figure is flexible and depends on the amount of improvement required. Therefore, the 2 hour figure above is a typical program. However, each program, including number of sessions per week is tailored to the needs of each student. The 2 hour session is standard to allow for sufficient instruction and practice and to ensure a consistent schedule during the week so that we can ensure that students have guaranteed availability when they come to C2.

Child & Adolescent Resource Education Services, LLC (CARES) - Monica Davis is the executive director and the primary tutor. She has eleven years experience as a teacher in public schools. Her tutoring experience includes facilitating an after-school tutoring program for two years at a school with students scoring below the state average on the IOWA and/or CRCT. Her personal achievements include raising her classes scores to above 60% in both years. She also was a facilitator and tutor of her church's volunteer tutoring program for grades three-high school. She utilized the Gwinnett County Academic Knowledge Skills to make lessons for the students. Carrter, Reddy & Associates utilizes Reciprocal Teaching strategies to tutor students in reading and mathematics. The Clarke County School District (CCSD) has been providing intense after school tutoring services for seven years. However this is the first time that CCSD has applied to be an SES provider. SES has been offered in Clarke County by outside providers for the last four years. Students that have participated in the district's afterschool tutorial programs, SES and others, have had a minimum of 5 to 10 point growth in reading or math state assessments scores. The structure of a CCSD SES tutoring session would include individualized plans tailored to the needs of each student developed by a certified teacher with 3 or more years of teaching experience. The sessions would include time for reteaching "hot standards" (those not mastered in previous assessments), as well as preteaching when possible. Instruction will be developed and modified to best meet the learning needs of each student. The tutoring session will include direct instruction (vocabulary development, skill teaching and practice, and computer intervention) using research-based materials and computer programs, such as Great Source, USA Test Prep, Walch Math, EXPRESS curriculum, Voyager Reading and Math, V-Math, and Fast ForWord, Renzulli, Successmaker, Math Moodle, Headsprout, and READ 180. These sessions would encompass meeting with students one to two days per week for 1.25-2 hours depending on the grade level. The sessions will be held in classrooms, computer labs, libraries, and in homes. Summaries of material covered in sessions will be maintained at the site-level and will be monitored by the site-based and district SES coordinators. Formative assessments (using the Scantron performance Series) will be given 2-3 times a year to help track students progress in the program and identify areas that need additional reinforcement. This assessment evaluates the student's scores and allows for identification of the domains/benchmarks where student needs academic support.

Progress occurs because of the Individualized nature of the program, while supplementing classroom work. Qualified tutors are “matched” to the student in Reading, Language Arts and/or Math. Parent insight, teacher feedback, school assessment scores, and initial pre-testing form a program blueprint, and tutoring progresses as subject mastery is achieved. Cool Kids Learn, a non-profit educational organization, has provided academic tutoring and enrichment programming for 7 years. Funded in part by various children's service councils and the United Way, our programs and services have as their primary mission to help children and youth build the academic skills and the love of learning they need to succeed in school and in life. We are currently operating SES tutoring in 10 states. We have a strong history of documented test score improvement: latest (2008-9) pre- and post-test performance data indicate that students who attended our program improved reading and math skills by up to 40%, with students at greatest risk showing some of the largest gains. Participating schools also have reported better overall engagement in the learning process. Tutors meet with students at their school or nearby community location (religious or community center, library) or in the student's home. The Dougherty County School System (DCSS) is an LEA (Local Educational Agency) is a State endorsed Supplemental Educational Services (SES) provider. An analysis of data regarding DCSS tutorial participants of the past three years indicate that over 400 participants passed the retest of the Reading CRCT (Criterion Referenced Competency Test) test, and that 70% of the participants passed one or more parts of the GHSGT (Georgia High School Graduation Tests) retests. Services will also address End of Course Tests (EOCT). SES services will be provided in the form of a Saturday Academy with a mid-week follow-up session. The weekly Saturday Academy will operate from 9:00 a.m. to 1:00 p.m., and the mid-week follow-up session will be held on Wednesdays from 4:00-6:00p.m. Students will be provided tutoring in Reading/Language Arts, and Math. Individual learning plans (ILPs) based on learning styles and weakness in GPS standards and content domains of CRCT/GHSGT tests respectively will be created for each student. By-weekly formal assessments and weekly informal assessments determine standards mastery. The results of these assessments will be included in the student progress reports and communicated to parents and students on a monthly basis. Students receiving SES services will have access to 21st Century Classroom technology and highly qualified (HiQ) teachers. Teachers will use differentiated instruction to address student needs. Participants will have access to computer labs with network and internet based software programs such as FASTMATH, I-Learn, and Learning Village, Individual Prescription for Achieving State Standards (IPASS), and Study Island. The system will maintain a tutor-pupil ratio of no greater than 1:8. So, the number of SES sites provided by the district will be determined by student participation. Program Summary
Dr. Al Science Education Services provides interesting and engaging science enrichment sessions that help 11th and 12th grade students understand concepts in biology, chemistry, and physics. A pre-assessment diagnostic, weekly quiz, home activity, post-assessment diagnostic, coupled with individualized learning activities are used to deliver the science enrichment curriculum and assess student learning. The sessions designed to help students pass the science portion of the GHSGT are exceptional. We helped students who failed the test previously achieve a passing score, and currently 100% of the students that complete these sessions pass the science portion of this test. We deliver 3 hours of instruction each week, which consists of 2 sessions per week, each lasting 1.5 hours. We require that at least 12 students in each LEA sign up for our services before we will begin the sessions. Dr. Al Science Education Services is in its third year as an SES provider and we remain committed to helping students understand science concepts. We enjoy helping students succeed in science and we constantly look forward to helping the next student(s).

Program Summary
Educate Online is America’s leading provider of live, personalized, at-home tutoring. Our programs in math and reading utilize individualized, direct instruction and mastery learning techniques to address each students specific skill gaps. Educate Online has successfully served more than 50,000 students since 2002, including more than 1000 in Georgia since 2007. On average, students who complete either our math or reading program gain at least a grade level. This also positively impacts in-school performance and grades. All instruction takes place on a computer and headset provided by Educate Online. Educate Online will also arrange internet connectivity, if needed. To ensure student safety, the computers are loaded with security software embedded in the operating system only allowing students to access the Educate Online program until they complete their tutoring. Students typically take two to four 60 minute sessions each week. Using headset and computer, they will be taught by a live instructor. All sessions take place in the safety and convenience of each student's home - eliminating the need for transportation. Sessions are available seven days a week and are taught by U.S. state-certified teachers with at least two years of teaching experience. Student-to-teacher ratios do not exceed 3:1. Students take an assessment test at the beginning of the Educate Online program to provide a baseline for determining an individualized learning plan. A Prescription Monitor reviews each plan to ensure that the skills are matched up properly and the curriculum is appropriate for that student. The plan presents content areas where the student is struggling, specific skills or strategies needed and examples of skills or strategies in use. Educate Online is able to serve ELL, special education, and disabled students given their ability to perform the necessary computer related functions.

Education & Guidance Services (EGS) has been providing the supplementary educational needs of the students of West Central Georgia for sixteen years. We are a SACS/CASI accredited special-purpose school and have been providing tutorial support for the NCLB program from its very first day. More than fifty dedicated, certified educators make up our team. Teachers are available, by appointment only, seven days each week. As educators, we all enjoy the delight in watching young people rekindle their love of learning. Our success for all students utilizes the one-on-one expertise of the tutor, skill-appropriate written material – not computers – to strengthen the student’s skill base, academic self-confidence, and classroom achievement. Students are given a thorough, initial diagnostic session involving math, reading, and language arts skills. The results are reviewed with the parent, student, and tutor; a plan of action is designed with goals proposed and tutorial activities planned using educational activities appropriate for the student’s needs. It is expected that the student will attend all scheduled (mutually agreed upon) sessions and will do follow-up reinforcement work at home between sessions. Most students meet with their tutor once or twice each week for one hour and most students watch their grades, standardized test results, and the CRCT or GHSGT scores really climb. Our approach with each student includes a knowledge that all students: have a special learning style, can achieve a delight in learning, can set and reach meaningful academic goals.

Educational Enterprises is a tutoring and consulting company that was founded by Dr. Glenna Tyler in 1999. We have provided SES tutoring for 7 school years beginning in 2002-2003.
Since 2002-2003 Educational Enterprises has been successful in assisting numerous schools in meeting AYP; Riley and Barden Elementary schools in Macon, Georgia and Rutherford Elementary in Detroit, Michigan. We have also assisted other schools in raising their student’s academic achievement on state standards; Weaver Middle School in Macon, Georgia, Campbell Middle School, Carson Middle School and Turner Middle School all in Atlanta, Georgia
Programs will be conducted at schools, churches or community centers for participating students. Sessions last from 1.0 hours to 2.0 hours per day with an average session being 1.5 hours. (Some Saturday programs run 3.0 hours 4 days per month). Sessions will run 2-3 days per week. Program hours will run 20-30 depending on school district.
All tutoring programs will begin with Achieve assessment test for K-12. After students strengths and weaknesses have been determined an individualized plan is developed to target specific areas and skill gaps for tutoring. Goals and objectives will be determined by identifying skill gaps and correlating them to state standards for mastery. Student progress will be monitored daily. A monthly progress report and a detailed tutoring log will be sent home. As students master goals and objectives this will be noted on the tutoring log, then new goals and objectives will be determined for mastery.
All materials will be provided by Educational Enterprises, if technology is needed for service delivery it will be available either at school sites, churches or community centers for all students.

EduTech Cognitive Therapy provides free on-line and off-line tutoring in reading and mathematics to K-12 students. Program Summary
Our program will use Direct Instruction, high participation, high interest curriculum materials. Assessments include pretest, posttest, and formative assessments. Pretest information is used to determine student's areas of weakness and to formulate the goals of the individual student plans.
Our program in Florida had a positive impact on 93% of the students who completed the program for reading/language arts and mathematics. 93% showed academic improvement. 91% demonstrated improved or good attendance. 95% demonstrated improved or good conduct and behavior.
Currently, 98% of the students have met the goals of the Individual Student Plan (ISP). 90% of the students who completed the program reported a postive impact. Tutoring sessions last up to two hours immediately after-school twice a week. Weekend tutoring is also up to four hours per weekend session. Tutoring sessions will be held at the school and curriculum materials will be provided.

The Empowerment of Minds (EOM) provides customized tutorial services for students grades K-12. In home tutoring, and online tutoring is available in Math, Reading, and English/Language Arts. EOM is designed to close academic gaps, develop test-taking strategies and assist in preparing students for the Criterion-Referenced Competency Tests (CRCT), Georgia High School Graduation Tests (GHSGT), and Georgia Alternate Assessment (GAA) state mandated test. EOM specializes in working with all identified students that qualifies under NCLB - Supplemental Educational Services including those with selected disabilities. In an effort to increase academic achievement, our program will assist in the development of the "total child". EOM small group sessions will be held at our learning center after school and on weekends with free transportation provided to selected sites. EOM fosters a safe secure environment that allows for academic growth, creativity, and character building. Our program combines technology with small group instruction to accommodate different learning modalities, along with flexibility in tutorial sites. Our tutors are certified teachers and other highly experienced educators. Students are given a pre and post assessment test to identify instructional level. Individual student skill gaps, strengths, and weaknesses are identified and are used to create a personal individual educational plan for each student. Research based best teaching practices are used to meet the individual needs of each student. Feedback is provided on student's academic progress. EOM will develop, nurture young learners, and positively influence their self-esteem. This will enable the students to have a renewed confidence and a sense of interest in learning to be successful in the classroom. Parents have reported a significant gain in grades, performance level growth on the CRCT, attendance, and self esteem as a result of their child completing EOM.

GA LEARNING UNLIMITED (GA L.U), providing tutoring services or five (5) years, uses the Diagnostic/Prescriptive Model, recognized widely as being highly effective, to increase student achievement. The program has a consistent record of assisting targeted students to improve in reading and mathematics an average of two (2) grade levels. The staff assesses the skills levels of each student. This information is used to determine those instructional goals and conditions necessary for each student's learning. The tutoring groups consist of 1-8 students with a certified or highly qualified teacher. Each student has a computer generated skills profile for both subjects. All instructional materials and classroom supplies are provided as a part of the service. The tutoring sessions are held after school, before school or on Saturdays. Research-based strategies and best practices will be used in the After School and Summer School LEAP sessions. For the past five years, Graham Consulting Group, LLC has offered In-home or small group tutorial programs to over 350 low-income students and families beyond the normal scope of school-based educational programs. Our tutorials have shown positive academic growth in several areas. For example, 90% of all students who have enrolled in our SAT/ACT preparation tutorials have achieved an increase of at least 250 points on their future standardized exams. In addition, 91% of all students who have participated in our Georgia High School Graduation Test tutorials have attained a minimal passing score in all subject areas. Moreover, 89% showed improvement in reading comprehension, while 95% showed improvement in Math. Our tutorials increase student academic achievement by providing a detailed curriculum that is aligned with the grade and subject specific Georgia Performance Standards (GPS). Our tutorial program will drive students to mastery in the following areas: reading, English, and mathematics. Students are tutored in one-on-one/small group settings following an instructional plan based on their strengths and needs. Using the students diagnostic test results, an individualized learning plan includes our recommended instructional program, based on the student’s individual needs. Our program is structured to give students 2-hour bi-weekly tutorials for a total of 15 weeks or 30 hours. The first and last two hours will be utilized for pre and post testing. Furthermore, instructors at Graham Consulting Group, LLC will have access to various technological resources including tape/CD players, grade appropriate books on tape, various listening activities and hands on manipulatives to assist in their tutorials. In 2002, in an effort to reach out and accelerate the achievement of at-risk students, the GFCDC established an educational remedial program to teach, mentor, and tutor elementary school children living in poverty, within the Metro Atlanta Public School System. Our tutorial services are interactive and focus on the core academic subjects of Reading, Mathematics, and Language Arts for grades 1 through 12. Our instruction will emphasize skills to help all students achieve school success and pass promotional test objectives that the state requires. Certified teachers and highly qualified tutors will work with students on a one-on-one basis and in small group settings to ensure student success. Our program offers personalized student learning plans, innovative and unique techniques to reach students, access to computer based learning, comprehensive assessments, weekly and monthly progress reports to parents and teachers, on-going evaluations, and nutritional snacks. Tutorial services will be held at the Greenforest-McCalep Academic Center which is convenient, safe, and conducive to learning. The tutorial schedule includes a Saturday, three-hour tutoring session from 9 a.m. until 12 noon. Our tutorial instruction and educational materials are consistent with Georgia Performance Standards as well as the National Reading Panel (NPR) and the National Council of Mathematics. They are high quality and based on the most current knowledge of what promotes student success. Our instructional techniques have been successful in helping students at a low-achieving Metro-Atlanta school pass the CRCT (standardized test) that resulted in the school being removed from the list of schools not making the state’s Adequate Yearly Progress (AYP). The Green Forest CDC was first approved as an SES provider for the 2003-2004 school year. Daughtry Elementary School provides instruction using a variety of instructional strategies designed to engage students and meet their needs by providing differentiated instruction based on indicdual needs. Needs are determined by a pre-test given to each student prior to enrollment in the program. Small group, one-on-one, and computer assisted instruction are used as models of instruction. Goals are set for the targeted student population to improve academic achievement. Bi-weekly progress reports are sent to parents to record student progress toward the goals. Results of the CRCT are used to determine the effectiveness of our program. During the school year, the program will be conducted after school from 3:40 pm to 5:40 pm for a two hour session. Tutoring will be offered two days a week. The National Reading Panel fully supports the fundamental concepts and instructional design of our materials. HireOrder Thinking Inc.(HOT) has over 3 years of expertise servicing students with reading and/or spelling deficits. HOT focuses on Reading, Spelling, and Language Arts instruction for grades 2-12 supporting NCLB, using certified staff, and following GPS standards. Wilson Reading System (WRS) addresses students with language-based learning disabilities, struggling readers and spellers, the "at-risk" population, and English as Second Language (ESOL) learners. WRS instruction component areas are: Reading-Phonemic segmentation, Alphabetic Principle-sound/symbol relationships, Decoding, Encoding (spelling), Advanced Word Analysis, Vocabulary Development, Sight Word Instruction, Fluency, Comprehension with Visualization, and Metacognition. Program Summary
I Can Kids, Inc. has been in the forefront of providing educational services for at-risk children for ten years. Services are currently being provided in Williamsburg, South Carolina at a small rural elementary school that has not met AYP for four years. Cades Elementary School Partnered with I Can Kids, Inc. to provide three hours after-school tutoring in reading, math, science, and social studies. I Can Kids, Inc. was awarded a 21st Century After-School Learning Center Grant to assist the struggling school meet NCLB and state academic standards. The students are making significant academic growth that the principal attributes to the after-school program. The after-school sessions are small groups of no more than ten students. All teachers are highly qualified. Tutors are utilized the direct instruction, hands on manipulative and the Leap-track reading program to build the skill level of the students. High interest reading books and other reading materials recommended by the highly successful “Great Source Reading Program” are incorporated in all instructional sessions. Summer skill building sessions incorporate reading and language and math skills in the arts, writing, and health and fitness. I Can Kids, Inc. have designed the after-school instructional program to reach struggling students. Monroe Elementary School (MES) Learning Enrichment Assistance Program (LEAP) has a seven year history of successfully tutoring struggling students. MES started LEAP in 2002 when it was designated as Needs Improvement. Research-based strategies and best practices were used in designing LEAP. With the help of LEAP MES is now a Georgia Title I Distinguished School - Year 2 and a 2008 Bronze Award Recipient for Greatest Gain in Student Meeting and Exceeding Standards. LEAP uses the research-based Teach Them to Read (TT2R) English/Language Arts and Reading curricula and research-based A+nyWhere (A+LS) Learning System: Mathematics Curriculum (Computer Assisted). LEAP SES tutoring meets twice a week for 1.5 hours a session for individual or small group tutoring. Overall the LEAP program structure, practice and procedure follows best practices as outlined in The Afterschool Training Toolkit, Quality Afterschool Learning. Specific product curricula and lesson plans are strictly followed by qualified and trained tutors. Students are assigned to English/LA, Reading or Math tutoring base on needs as determined by CRCT, ITBS, EOCT and other school assessments. Students receiving English/LA tutoring are given an assessment to determine specific areas of need. Students receiving Math take a computerized assessment. The A+LS provides a prescriptive course of study based on the assessment. As a SES provider since 2006, our company has served students in the areas of English, mathematics, science, and social studies. We have had great success in helping students pass the CRCT, GHSGT, EOCT, SAT, and ACT. The tutorial programs are very structured and take up to 30 hours to complete. Students usually attend 20 sessions two days per week for 60-90 minutes per session.

Mainly Math provides quality tutoring in all subjects allowed by the SES program. The owner and lead tutor has been tutoring over 20 years and has taught in both public and private schools. Mainly Math has been providing tutoring for SES for 5 years. We provide proven research based quality instruction for our students including the latest technology and teaching materials. I have over 5 years of experience tutoring students in subjects ranging from math to physics, This will be my first year applying as an SES Provider but have tutored for an SES Provider for the past two years.
Tutors will understand area(s) of weaknesses of the students by first assessing grade level test (Sample test included for 6th, 7th, 8th , and high school students). Then tutors will setup tutoring time on a one on one basis and will work on area(s) of improvement and will provide worksheets that are in line with the Georgia Mathematics Standards. Tutors will also help set goals with individual students to instill confidence, while monitoring their grade throughout the tutoring process. Tutors will also keep in contact with student’s teacher so that the teacher, tutor, and parent(s) are synch with the effectiveness of the program.
The length of the tutoring sessions will be an hour each week at the student’s home or at a local community center (i.e. library, school, etc.) Model of instruction will be on a one on one basis and goal setting will be taught from the initial meeting of tutor and student. Studies show good readers make better history, language arts, science, and even math teachers. The JRN curriculum applies this philosophy in the "reading across curriculum" technique which is prevalent throughout their material. NGA also believes that every child is unique. Students who attend NGA are individually assessed and a customized academic plan is implemented for each student. A self-paced curriculum that incorporates one-on-one tutoring with supplemental group activities is used. NGA has a proven track record of significantly improving student academic performance. Many students who completed the program have gone on to receive various scholarships including the HOPE Scholarship. In addition to our dedication to each student, we credit our students' achievements to the commitment to the program and parent support. We welcome the opportunity to work with you in helping your student pursue academic success. Specific tutoring times will be worked out between parents and tutor, however, your child's success will require that we meet at least 2 to 3 times per week with sessions lasting 1 to 2 hours.
